PUBLIC HEALTH NURSING
Public Health Nursing- Health Maintenance
Includes teaching and intervention to patients to manage chronic conditions and prevent acute exacerbations of disease processes. Nursing interventions will include medication set up, assessing medication compliance, blood pressure/blood sugar monitoring, venipuncture, general assessments, health /nutrition education and communication with physicians and other health professionals.
Public Health Nursing – Disease and Disability
Includes provision of skilled nursing services related to specific medical diagnosis and covered under a signed physician’s plan of treatment. Skilled services may include wound care, venipuncture and medication administration. In addition, teaching and training activities to teach a patient, their family or caregivers how to manage the treatment regimen may be provided.
Foot Care Clinics
Includes promoting health and wellness through assessment, education and care of the feet.
These service are provided through a subcontract with Wesley Community Services
